Care Model© Training

The Health Net Care Model© training series is focused on developing core skills for professionals in connector roles to build their knowledge and competencies through interactive sessions. These sessions are designed for cohort-based learning where participants share best practices and experiences in a group setting and complete learning objectives together.

What to Expect

As a participant in the Care Model© training, you can expect a fully interactive learning environment that is supportive and challenging, allowing you to push past your learning edge toward growth. The Care Model© training series incorporates self-care and trauma-informed activities, as we recognize the effects of secondary traumatic stress on helping professionals. Intentionally designed to be completed sequentially to reinforce concepts, the full series consists of 16 hours of training facilitation. Our expectation of participants is to enter the space fully present for all sessions and activities. Eligible participants can receive up to 13 CEU credits for social workers and 16 CEU credits for Community Health Workers upon completion of the cohort.

Session Descriptions

Empowerment Approach and Skills 

This session teaches empowerment as a practice approach embedded in the strengths perspective. Empowerment is not something we do to clients but with clients as we walk alongside them while they use their own voice and make their own choices.

 

Professional Boundaries

This session teaches the guidelines necessary to both protect clients from harm and preserve the mental well-being of staff members. Staff members also learn to navigate gray areas and develop healthy working relationships.

Health Equity 101 

This session provides a foundation for understanding health equity and health-related social needs and a baseline introduction to personal identity and the value of inclusion in everyday practice.

 

Best Practices for Working with Interpreters

This session shares national standards and widely recognized best practices for language assistance; one of the easiest ways to reduce barriers within systems.

Personal Interview & Client Action Plan

This session teaches the personal interview and client action planning process while using motivational interviewing skills. Practice open-ended questions, affirmations, reflections, and summaries to elicit the client’s story while conducting an empowering interview and planning session focused on client goals.

Strengths-Based Documentation

This session covers the skills and intentionality needed to identify and affirm strengths in clients and their environments and reflect this in professional documentation.

 

Community Resource Navigation

This session introduces various community resource directories, databases, systems, and collaboration methods to increase knowledge of community resources and referral options. This reduces duplication of efforts, improves efficiency, and ensures the accuracy of referrals.
