Our Why
At Health Net, we imagine a community where we are all healthy, have a safe place to live, an income that meets our family's needs, and resources that support health and wellness. We all have access to nutritious food. We all can obtain health insurance and afford the services that we need to thrive. We believe that living a healthier life should not be complicated. We approach this work from a spirit of simplifying the “hard stuff” by making being healthy easier for people to access. We believe in the power of individuals, communities, and systems working together to drive better health outcomes.
Our Expertise is Community Health
Since its inception, Health Net has been charged with demonstrating success through scalable interventions that integrate multiple levels of provider and community engagement. Our earliest demonstration project, the Children’s Healthcare Access Program (CHAP) in Kent County, has grown with the needs of communities and has been implemented in various counties in Michigan through the MI-CHAP pilot. As a result of the ability to scale our approaches, Health Net partnered with the Centers for Medicare and Medicaid Services (CMS) to become an “Accountable Health Community” and implemented a universal social needs screening process in clinical and community sites in Kent County.
During this project, we refined our approach to navigation, and codified this method as our “Care Model©.” Through the Care Model©, Health Net forms relationships with primary care providers and works directly with families to help strengthen their connections with healthcare by addressing social determinants of health (SDoH) needs. Our staff is credentialed with experience working with individuals in the community. Our team partners with other aligned organizations to advocate and advance system changes. Our work centers on producing strong outcomes that drive change at the policy systems level for all.
Individuals
We invite individuals to share their stories and experiences while opening doors for people to participate and lead in decision-making. This is demonstrated through client advisory roles and connecting our advisors to community leadership opportunities.
Communities
We can take a concept, develop the model, and scale to operability. Through collaboration, coordination, and streamlined processes, we partner with community organizations to broaden our reach.
