A Foundation for Professional Growth and Development
At Health Net, we value education and making sure our staff is equipped with the skills and confidence to effectively help clients. This extends to our community training programs where together, we believe a competent and confident workforce is the link to providing quality care. Often, these positions are filled by first time workers or newly promoted staff members. Building upon our own experiences, research, and best practices, our training provides a foundation for professionals in either a “helper” or supervisory role to feel more empowered to offer support and use strategies to create rapport and deliver high quality services.
Is Our Training Right for You?
Designed to enhance your skills in guiding patients, clients, and community members through navigating complex systems and problem solving, Health Net of West Michigan has built training based on expertise in community navigation, supervision, and client advocacy experiences. From human services professionals like community health workers and case managers to frontline coordinators to those working with specialized groups; our training is designed for professionals that connect clients to resources, coordinate services across organizations or teams, and believe that everyone has strengths and can be part of the solution.

Care Model© Training
This training is a foundational course that builds baseline skills for those in helping roles to work from. The goal of this course is for participants to leave ready to deliver high quality care and services to clients.

License Our Curriculum
Are you an organizational leader who wants to provide a framework for your team to effectively serve clients? We offer licensing agreements for Care Model© materials to support teams that have completed training, and for organizations to facilitate training across their workforce.

Supervisor Learning Cohort
Are you a new supervisor or an established supervisor who wants to add skills to your toolbox? Do you want new approaches to support your team? We’ll help you work through your leadership goals in our Supervisor Learning Cohort.

Our training format allows for collective expertise and wisdom to be shared among peers to be modeled in real time; this helps participants practice the skills multiple times prior to completing the course and build the confidence to use them in the field. We are building a community of practice with shared knowledge, understanding, and standards to impact the health of our community.
What is Success?
Upon completion of our training, you gain an expanded network of peers working towards similar goals, deepened confidence in your skills and abilities to serve clients, new strategies to support your staff, and CEU credits, if applicable.

Meet our Facilitators

Christina Pavlak, LMSW (She/Her)
Christina is the Vice President of Training and Development at Health Net and is an experienced supervisor, nonprofit leader, and instructor with a personal passion for the practice of supervision. She builds effective teams, and designs and implements client-level services and staff training to improve community health.

Shannon Kaza (She/Her)
Shannon is the Training Project Manager at Health Net of West Michigan. She is also a certified trauma-informed yoga instructor and has been teaching mindful movement practices at Grand Valley State University since 2001. With a strong foundation in health and wellness, Shannon is passionate about creating inclusive spaces that foster empowerment, awareness, and personal growth.

Vanessa Jimenez (She/Her)
Vanessa is an experienced nonprofit leader and diversity, equity, and inclusion educator who interconnects her education and her lived experiences as a Latina navigating dominant-culture spaces in advocating for marginalized communities and working to dismantle systems of oppression and inequities.
