We work from a strengths perspective, with an empowerment approach, while bringing into focus the role equity plays in the successes of our community. We believe that every individual, group, family, and community has strengths and we assist people to use their strengths to overcome their challenges. Throughout the process, we strive to dismantle inequitable systems in our workplace and community.
Our team helps our community to connect to:
- Early childhood services and prenatal resources
- Providers who specialize in health services, dental, and behavioral health
- Healthcare coverage options
- Transportation options for healthcare visits
- Community resources including housing, food, utility assistance, and many more
What is healthcare and social services navigation?
At Health Net, navigation means helping people access and effectively use healthcare, community services, and informal support networks. Our navigation services are like the GPS maps on your phone, but personalized to your strengths, needs, concerns, goals and plans so you can be healthy and successful.
Our team can work with you, much like a GPS, to figure out different paths or options, guide you through an area you are unfamiliar with, talk to you about potential barriers or obstacles that might get in the way of your plan, estimate the amount of time your selected path might take, suggest a backup plan if you change your mind along the way, and can tell you about other resources in the area.
Community Healthcare Access Program (CHAP)
CHAP assists adults and children in maintaining their health and understanding how to navigate the healthcare and social service system.
