De hecho, estos factores explican casi la mitad de las variaciones en los resultados y desaf\u00edos de salud que enfrenta nuestra comunidad. <\/p>\n<p>[\/vc_column_text][vc_single_image media=\u00bb7955&#8243; media_width_percent=\u00bb100&#8243; alignment=\u00bbcenter\u00bb shape=\u00bbimg-round\u00bb radius=\u00bblg\u00bb uncode_shortcode_id=\u00bb243369&#8243;][vc_column_text uncode_shortcode_id=\u00bb642228&#8243;]<\/p>\n<p class=\"p1\"><strong><span class=\"s1\">La Soluci\u00f3n: El SDoH Hub del Condado de Kent <\/span><\/strong><\/p>\n<p class=\"p1\">Para enfrentar esta creciente problem\u00e1tica, el Condado de Kent est\u00e1 promoviendo la colaboraci\u00f3n entre los sistemas de salud y los proveedores de servicios sociales. La creaci\u00f3n del Centro de Determinantes Sociales de la Salud (SDoH Hub) representa un paso innovador hacia un enfoque m\u00e1s conectado, integral y humano en la salud comunitaria. Para Health Net of West Michigan, esta iniciativa se construye sobre nuestro trabajo de los \u00faltimos cinco a\u00f1os como parte de la Colaboraci\u00f3n Regional de Salud del Condado de Kent. Seguiremos desempe\u00f1ando un papel clave en el desarrollo de este nuevo centro, con la meta de establecer los siguientes componentes esenciales:  <\/p>\n<p class=\"p1\" style=\"padding-left: 40px;\">1. Toma de Decisiones Compartida: Involucrar activamente a miembros de la comunidad y organizaciones locales en la toma de decisiones para garantizar que las voces de quienes m\u00e1s enfrentan desigualdades sean escuchadas y priorizadas.<\/p>\n<p class=\"p1\" style=\"padding-left: 40px;\">2. Aprendizaje y Evaluaci\u00f3n: Dise\u00f1ar, evaluar y mejorar pol\u00edticas y programas que permitan a las personas y comunidades prosperar.<\/p>\n<p class=\"p1\" style=\"padding-left: 40px;\">3. Sostenibilidad: Unir fondos de diferentes fuentes para atender necesidades identificadas por la comunidad de forma sostenible.<\/p>\n<p class=\"p1\" style=\"padding-left: 40px;\">4. Participaci\u00f3n Comunitaria: Incluir de forma continua a una comunidad diversa durante todas las fases de planeaci\u00f3n e implementaci\u00f3n para asegurar un enfoque aut\u00e9ntico y efectivo.<\/p>\n<p class=\"p1\" style=\"padding-left: 40px;\">5. Gobernanza s\u00f3lida: Establecer criterios y estrategias claras para las alianzas es esencial para lograr mejoras sostenibles en los resultados de salud.<\/p>\n<p class=\"p1\" style=\"padding-left: 40px;\">6. Infraestructura Robusta: Desde capacitaci\u00f3n del personal hasta la creaci\u00f3n de un Intercambio de Informaci\u00f3n Comunitaria y procesos de facturaci\u00f3n compatibles con HIPAA, la infraestructura fortalecer\u00e1 el enfoque coordinado para reducir desigualdades en la salud.<\/p>\n<p class=\"p1\" style=\"padding-left: 40px;\">7. Recopilaci\u00f3n y Evaluaci\u00f3n de Datos: Recolectar datos \u00fatiles para fundamentar las estrategias de equidad en salud y fomentar la mejora continua.<\/p>\n<p class=\"p1\"><span class=\"s1\"><strong>Primeros Pasos y Retroalimentaci\u00f3n Comunitaria<\/strong>  <\/span><\/p>\n<p class=\"p1\">Este esfuerzo comenz\u00f3 en Noviembre de 2024, cuando nos unimos con el Essential Needs Task Force del Condado de Kent, en colaboraci\u00f3n con Heart of West Michigan United Way y el Departamento de Salud del Condado de Kent, para organizar el evento \u201cEl Poder de la Conexi\u00f3n: Dando Forma al Centro de Determinantes Sociales de la Salud del Condado de Kent.\u201d Con la participaron 115 representantes de organizaciones sociales y sistemas de salud, surgieron varios temas clave: <\/p>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul class=\"ul1\">\n<li class=\"li1\">Fuerte inter\u00e9s en la participaci\u00f3n comunitaria en la planeaci\u00f3n y gobernanza<\/li>\n<li class=\"li1\">Necesidad de coordinaci\u00f3n entre sistemas y tipos de servicios<\/li>\n<li class=\"li1\">Inter\u00e9s en equilibrar las necesidades de organizaciones grandes y peque\u00f1as<\/li>\n<li class=\"li1\">Preguntas y preocupaciones sobre el uso compartido de datos y el financiamiento<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p class=\"p1\"><strong><span class=\"s1\">Pr\u00f3ximos pasos  <\/span><\/strong><\/p>\n<p class=\"p1\">Actualmente, un grupo de planificaci\u00f3n trabaja en propuestas de financiamiento para seguir avanzando en el proceso comunitario. Ya existen fondos a corto plazo del Departamento de Salud del Condado y de Heart of West Michigan United Way para apoyar los primeros esfuerzos, y se est\u00e1n buscando m\u00e1s oportunidades de financiaci\u00f3n.<\/p>\n<p>Los pr\u00f3ximos pasos incluyen:<br \/>Crear un modelo l\u00f3gico preliminar para evaluar recursos y resultados deseados<\/p>\n<p>Establecer reuniones con l\u00edderes de sistemas de salud<\/p>\n<p>Organizar una reuni\u00f3n virtual en verano de 2025 para recolectar m\u00e1s ideas<\/p>\n<p>Formar grupos de trabajo para ampliar la participaci\u00f3n y asegurar voces diversas<\/p>\n<p>A medida que el Condado de Kent avanza con estas acciones, el camino hacia la equidad en salud se vuelve m\u00e1s claro. A trav\u00e9s de los esfuerzos coordinados del SDoH Hub, estamos construyendo un futuro m\u00e1s saludable y equitativo para todas las personas.
