Este modelo est\u00e1 transformando la manera en que nuestros gestores de casos, junto con organizaciones aliadas, se relacionan con las personas a las que atienden\u2014particularmente aquellas que enfrentan m\u00faltiples desaf\u00edos para acceder a recursos esenciales. Al reconocer las necesidades \u00fanicas de cada poblaci\u00f3n seg\u00fan su ubicaci\u00f3n, nuestro enfoque busca aumentar la participaci\u00f3n y eliminar barreras que muchas veces impiden el acceso al apoyo necesario. <\/p>\n

Un componente clave de este modelo es la presencia de una o un gestor de casos directamente en la organizaci\u00f3n aliada.
