Juntos, trabajamos para aumentar la capacidad de navegaci\u00f3n en salud comunitaria de forma cultural y ling\u00fc\u00edsticamente relevante, basada en buenas pr\u00e1cticas y con un enfoque centrado en la equidad y el empoderamiento. Nuestro objetivo a largo plazo: reimaginar c\u00f3mo pueden colaborar las organizaciones del condado de Kent que realizan un trabajo similar (como navegaci\u00f3n de recursos mediante promotores de salud y trabajadores de salud comunitaria, compartiendo conocimientos, apoy\u00e1ndose mutuamente y aumentando el acceso a recursos para las personas de la comunidad mediante una red fuerte de acci\u00f3n colectiva.<\/p>\n<p class=\"p1\"><strong>Antecedentes<\/strong><\/p>\n<p class=\"p1\">Durante la pandemia de COVID-19, el KCHD recibi\u00f3 financiamiento a trav\u00e9s de la iniciativa Advancing Health Literacy del Departamento de Salud y Servicios Humanos de los Estados Unidos. Los fondos llegaron en Julio de 2021 y el proyecto comenz\u00f3 en Octubre de ese mismo a\u00f1o. Seis organizaciones de base comunitaria fueron identificadas con el objetivo de desarrollar y trasmitir mensajes cultural y ling\u00fc\u00edsticamente apropiados sobre la COVID-19. A medida que la pandemia fue estabiliz\u00e1ndose, esta iniciativa evolucion\u00f3 hacia una nueva fase, ampliando el trabajo para ayudar a los miembros de la comunidad con los complejos sistemas que abordan los determinantes sociales de la salud.<\/p>\n<p class=\"p1\"><strong>Siguiente Fase<\/strong><\/p>\n<p class=\"p1\">Health Net of West Michigan fue incorporado para dar continuidad a este trabajo y apoyar la creaci\u00f3n de una comunidad de pr\u00e1ctica que colaborara con las organizaciones comunitarias para abordar las barreras de acceso y las brechas en la atenci\u00f3n. Al mismo tiempo, se busc\u00f3 construir una comunidad de aprendizaje, experiencia y conocimientos compartidos sobre el \u201cqu\u00e9\u201d y el \u201cc\u00f3mo\u201d de la navegaci\u00f3n como pr\u00e1ctica.<\/p>\n<p class=\"p1\">Las metas acordadas fueron:<\/p>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul class=\"ul1\">\n<li class=\"li1\">Identificar nuevas fuentes de financiamiento sostenible para conservar los puestos de navegadores<\/li>\n<li class=\"li1\">Co-crear y estandarizar procesos, conocimientos y habilidades para expandir el alcance y experiencia de las y los navegadores<\/li>\n<li class=\"li1\">Fortalecer la capacidad de las organizaciones comunitarias participantes<\/li>\n<li class=\"li1\">Crear una red de organizaciones sostenibles que utilicen buenas pr\u00e1cticas para la navegaci\u00f3n de servicios relacionados con los determinantes sociales de la salud<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p class=\"p1\"><strong>\u00c9xitos Compartidos<\/strong><\/p>\n<p class=\"p1\">Como red, pudimos utilizar la financiaci\u00f3n trenzada para aprovechar las alianzas y lograr un impacto colectivo, al tiempo que permit\u00edamos a las organizaciones de base comunitaria decidir c\u00f3mo quer\u00edan utilizar sus fondos dentro de su organizaci\u00f3n para satisfacer sus necesidades Tambi\u00e9n desarrollamos acuerdos comunitarios compartidos, que construyeron las bases para una comunicaci\u00f3n transparente, confianza mutua, aprendizaje y crecimiento. Como colectivo, mantuvimos muchas conversaciones sobre nuestras distintas organizaciones, lo que hacemos y a qui\u00e9n servimos, que ampliaron la conciencia cultural de cada socio sobre nuestras diversas comunidades.<\/p>\n<p class=\"p1\">Uno de nuestros logros m\u00e1s tangibles fue el fortalecimiento de la capacidad de los navegadores comunitarios para ayudar a miembros de la comunidad a navegar los sistemas complejos y, a menudo, inaccesibles que afectan su salud y bienestar. Esto se hizo mediante la ense\u00f1anza de la formaci\u00f3n Care Model\u00a9 de Health Net of West Michigan y el posterior modelado entre nuestros gestores de casos y los navegadores de nuestros socios.<\/p>\n<p class=\"p1\"><strong>Situaci\u00f3n Actual<\/strong><\/p>\n<p class=\"p1\">Debido a la reducci\u00f3n de recursos, la iniciativa se transform\u00f3 en una red formal de coordinaci\u00f3n.
