Our partnership with Catherine’s Health Center officially launched in 2023, creating new opportunities to work alongside their team at Streams Community Health Clinic, located in Kentwood. This place-based collaboration brings multiple services together in one accessible space for community members. 

At the Streams location, Catherine’s Health Center team provides medical, dental, and behavioral health services, while Streams supports individuals and families through educational programming for students in kindergarten through 12th grade and supplemental food programs. Together, these models ensure that community members can address both health and social needs in one place.   

The partnership has been built on a strong foundation of collaboration and open communication about patient needs. Our resource navigator shares that they feel warmly welcomed by the clinic staff and have become a natural extension of the care being provided there, helping connect patients to resources that support their overall well-being.  

Impact on the community in action 

Albert’s* story is one example of this partnership in action.   

Albert, a patient at Catherine’s Health Center, shared that he needed additional support as he approached retirement age and didn’t have a working phone. Since he lived near Streams, he was able to walk to meet with one of our on-site resource navigators. Together, they completed a phone appointment with the Social Security Administration to set up Social Security benefits.  

Thanks to the convenience of walking to Streams and receiving in-person support, Albert successfully completed the process and is now receiving $1,000 in additional income each month, a meaningful boost that supports his financial stability as he enters retirement.
