FitKids360 is a seven week pediatric obesity intervention program for overweight and obese children with a BMI at or above the 85th percentile for their age and gender. Children and their families are referred to the program by their physician. FitKids360 focuses on teaching children and their families about living a healthy lifestyle, rather than focusing on weight loss. Learn more

Community Healthcare Access Program

CHAP assists adults and children to maintain their health and understand how to navigate the healthcare system, thus reducing costs and improving health outcomes. The program provides educational resources, case management for chronic illnesses and addresses social determinants of health (SDoH). CHAP is open to members on participating Medicaid health plans and uninsured.  Learn more

Kent County Oral Health Coalition

KCOHC is comprised of medical and dental professionals, community agencies, and funders working together to ensure access to affordable oral health care and knowledge on basic oral health care throughout the life spectrum from infants to senior citizens. Learn more

Accountable Health Communities

Accountable Health Communities Model is funded through the Centers for Medicare & Medicaid Services to create a centralized screening and referral system between formal healthcare systems and community organizations to address social determinants of health. Learn more
