Funding & Partners
We help bridge gaps in care by identifying existing resources in our community and supporting our partners’ efforts to improve access to health care and social needs services. We are a neutral organization that brings key stakeholders together from formal healthcare systems, community agencies, governmental bodies, Medicaid health plans, and the community at large. Through these efforts, we have become an important voice at many decision-making tables, helping to lead system-level initiatives including the Accountable Health Communities Model.
