Community Health Worker

Health Net of West Michigan has a full-time opening for a Community Health Worker. Duties include conducting telephone intake of new clients, assisting clients with access to healthcare and health-related social services by phone and in person, providing telephone consultation, as needed, and following up with clients to verify services were obtained. The ideal candidate will be passionate about helping people, knowledgeable about resources and social services available within the community and able to work with a high degree of advocacy and confidentiality. Client Services Supervisor

Health Net of West Michigan has a full-time opening for a Client Services Supervisor. Duties includes supervision of assigned Client Services Staff, performing case management activities and maintaining a client case load. This valued individual will conduct research into best practices as they relate to Health Net of West Michigan programs and initiatives, assist in the development of policies, procedures, and outcomes and collaborate with Client Services Director to provide guidance and direction to the team.
