Intake Internship

Health Net of West Michigan has openings for Intake Internships. Health Net seeks positive, self-motivated individuals with high quality standards to work in an innovative and diverse environment. The intake internship provides an excellent opportunity for practical experience in customer services, data entry, and program operations support. While under the supervision of the Intake Supervisor, the Intake Intern will have the opportunity to assist clients and community partners by serving as the entry point to Health Net of West Michigan's services, and routing referrals to appropriate staff for service delivery. This position requires a high attention to detail in data entry and data collection. The Intake Intern will work as a member of the Client Services team that provides services to individuals/ families by addressing social determinates of health. The Internship Supervisor will conduct regular meetings in which the intern can share their experience to assure that intern is participating in a meaningful work experience. Learn more here.

AHC Site Coordinator

Health Net of West Michigan has a full-time opening for an Accountable Health Communities (AHC) Site Coordinator who is directly responsible for supporting and engaging the AHC screeing site partners. Key responsibilities include creating and monitoring clinical site profiles; analyzing and distributing screening site data reports; evaluating the screening process and quality improvement efforts; maintaining all site materials and supplies; interacting with senior internal and/or external personnel on significant matters; and supporting the mission, vision, values and key strategies of Health Net and the organizations with which it interfaces. This position will work as a member of an interdispilary Health Net program operations team, supporting Health Net's efforts to assist individuals/ families address Health Related Social Needs (HRSN), and to acheive health equity for individuals/ families.
